Tourist visas for Myanmar, with a 28 day validity, are available from any Myanmar Embassy or Consulate worldwide. These visas cannot be extended.
It is possible to get a visa on arrival at Yangon airport as long as you have clearance from a Myanmar tourism company. It is far cheaper to get the visa from an embassy.
We have had multiple reports from people that you can get a visa on arrival at Yangon airport without any pre-clearance. Getting on the plane to do this may be difficult.

The borders with Thailand are open, you will need a visa in advance if you intend to arrive from Thailand.
The borders between India and China are somewhat open. You will need to apply to a consulate for permission at least a month or two in advance (They need approval from Yangon) and give a full itinerary of what you plan to do. It is recommended that you keep the application and what you plan to do simple, and if you would like to make any changes then do them in Yangon. You can also apply for permission in Yangon which will take two or three weeks to process. Crossing by land will most likely cost more than flying.
